  • Megjegyzések: PURPOSE: The aim of the study is to investigate the infertilityrelated stress in a Hungarian infertile population and examine the effects of gender roles, child wish motives, subjective well-being, and marital relationship on the experience of infertility according to our self-constructed conceptual framework. METHODS: Validated self-report questionnaires measuring the factors of the conceptual framework were taken in the study carried out in a sample of 53 people attending the fertility unit of a Hungarian clinic. RESULTS: Infertility-related global stress, infertility-related social concerns, and general health problems have more intensive effect on women than on men (all p < 0.05). Women from the infertile group scored higher their femininity (p < 0.001) and lower their general health (p < 0.05) than the reference population. Infertile men believe deeper in meaning of life than women (p < 0.05) or reference population (p < 0.01). Femininity (ß = 0.460, p < 0.05), traditional gender role concepts (ß = -0.248, p < 0.05), general health (ß = -0.474, p < 0.05), and marital relationship (ß = -0.251, p < 0.05) play the strongest role to predict stress caused by infertility. CONCLUSIONS: The current study emphasizes the importance of interrelations of gender role attitudes, gender role identification, general health, and satisfaction in couple relationship with infertility-related stress. In further investigations, both social and personal aspects and their effect on experiencing infertility need to be measured in infertile people, particularly in different cultural settings.

  • Megjegyzések: The aim of this study is to evaluate the relationshipof serum sclerostin levels with age, cystatin C,bone mineral density (BMD) and biochemical markers ofbone turnover in healthy Hungarian men[50 years of age.We determined serum levels of sclerostin and examinedits relationship to age, cystatin C, osteocalcin, C-terminaltelopeptides of type-I collagen, procollagen type 1 aminoterminalpropeptide, 25-hydroxyvitamin D, parathyroidhormone, and L1?L4 (LS) and femur neck (FN) BMD dataavailable from 194 randomly selected ambulatory menbelonging to the HunMen cohort. In the study population asa whole [n = 194; age (median, range) 59 (51?81) years],statistically significant correlation was found betweensclerostin and age (r = 0.211; p = 0.003), cystatin C(r = 0.246; p = 0.001), FN BMD (r = 0.147; p = 0.041)and LS BMD (r = 0.169; p = 0.019). Compared tomiddle-aged men (age B59 years, n = 98), elderly men(age [59 years, n = 96) had significantly higher serumsclerostin levels (67.8 ? 15.9 vs 63.5 ? 14 pmol/L; p =0.047). Among men with normal (T score [-1.0) FNBMD, the elderly had significantly higher serum sclerostinlevels compared to the middle-aged men (70.4 ? 17 vs63.9 ? 11.5 pmol/L; p = 0.019). Furthermore, among theelderly men cystatin C was the only significant predictor ofserum sclerostin levels (standardized regression coefficient(b) = 0.487; p\0.001). In the studied healthy elderlycohort, this study reports a significant increase in sclerostinlevels with increasing age and deteriorating kidney functionas determined by plasma cystatin C levels.

  • Megjegyzések: Previous studies have dealt with maternal blood rheology in preeclampsia (PE), but only a few focused on the fetal rheological parameters in this maternal state. PE is one of the most common severe complications of pregnancy world-wide associated with high maternal morbidity and mortality and intrauterine fetal growth restriction. Our objective was to explore the rheological parameters in the umbilical cord blood at term in the presence of moderate and severe forms of PE.METHODS:A subgroup analysis was performed in a community based retrospective study of 4,951 consecutively recorded singleton pregnant women of whom 423 had PE. In the latter, umbilical cord blood was collected during delivery for testing of hematological and blood rheological parameters of their fetus. Fetal results from uneventful pregnancy were compared with those associated with preeclampsia. Furthermore, results were also evaluated in the presence of moderate and severe forms of PE. Plasma viscosity (pv) was examined using KSPV 1 Fresenius and Red Blood Cell (RBC) aggregation (stasis: E0 and low shear: E1) using MA1-Aggregometer; Myrenne.RESULTS:According to the definition of the German Society of Gynecology and Obstetrics (DGGG) 314 women had moderate (74.2%), while 109 had severe forms of PE due to the presence of a blood pressure > 170/110 mmHg (n = 41; 9.7%), and/or IUGR < 5th percentile (n = 28; 6.6%), and/or HELLP-Syndrome (n = 10; 2.4%), and/or proteinuria ? 5 g/24 h (n = 30; 7.1%). When comparing the fetal results from pregnancies with and without preeclampsia mean hemoglobin values (p < 0.001) and hematocrit (p < 0.001) were markedly higher, while plasma viscosity (p = 0.006) and erythrocyte aggreagtion (stase: p = 0.35; low shear: p = 0.08) were lower in association with preeclampsia. Gestational age, fetal birth-weight and umbilical arterial cord blood pH in women with severe PE was statistically significant lower as compared to those with moderate disease (p < 0.001). Mean hemoglobin level and hematocrit were higher in fetus from women with severe compared to moderate PE, while plasma viscosity (1.03 ? 0.07 mPas vs. 1.05 ? 0.07; p = 0.05) and erythrocyte aggregation in stase (2.3 ? 2.47 vs. 2.41 ? 2.46; p = 0.11) as well as under low shear (7.86 ? 4.63 vs. 8.06 ? 4.60; p = 0.15) were lower. HELLP-Syndrome was associated with the lowest plasma viscosity (1.00 ? 0.07 mPas; p = 0.019) and erythrocyte aggregation (low shear: 5.1 ? 5.0; p = 0.04) in fetus. CONCLUSION:The results of this study including a notable number of patients with PE and their newborns revealed an in part statistically significant association between variables of blood rheology and the presence, severity and type of preeclampsia with a trend towards hyper viscosity in severe forms of preeclampsia.

  • Megjegyzés: Rheological blood parameters of neonates are different form those of adults. Many authors have studied changes in blood rheology in neonates in different clinical disorders. To-date, no one set the normal values for blood rheological parameters in healthy neonates. The aim of this study is to set the norm for rheological blood parameters in healthy newborns and to describe the changes in those parameters in common clinical disorders that affect the newborns. We recruited all the neonates born to mothers experiencing un eventful pregnancies, blood was taken from the umbilical cord right after the delivery. In this time period we recruited 4985 neonate. From this huge database we were able to set the standards for blood rheology in neonates, namely plasma viscosity of 1.06 ? 0.072 mPa, erythrocyte aggregation at stasis of 2.41 ? 2.74 s-1 and erythrocyte aggregation under low shear forces of 8.51 ? 6.38 s-1. These values changed significantly in some diseased neonates. This is the largest study investigating normal rheological parameters and deviations from the norm in common clinical disorders occurring in this early stage of life.

  • Megjegyzés: About one-third of human papillomavirus (HPV) types infect the anogenital tract. High-risk genital HPV types (such as HPV 16, 18, 31, 33, and 35) are linked causally to the development of cervical cancer. The long control region (LCR) of the HPV genome regulates the replication and transcription of the viral genome. In this study, the functional significance of nucleotide sequence variation within the LCR of HPV 31 was investigated. The LCR was amplified by polymerase chain reaction (PCR) from 41 HPV 31 positive cervical samples of Hungarian women. A phylogenetic tree constructed from the nucleotide sequences of the LCR variants revealed the presence of three intratypic variant lineages of HPV 31, in accordance with previous results. In order to explore the functional consequences of sequence variation in the LCR of HPV 31, selected LCR variants were cloned into a luciferase reporter vector, transfected into C33-A cells and tested in luciferase reporter assays. Significant differences were found between the transcriptional activities of HPV 31 LCR variants belonging to different variant lineages. As the LCR is governing the transcription of the E6 and E7 oncogenes, the differences in the transcriptional activities of LCR variants may be associated with differences in their oncogenic potential.

Men with 25-hydroxyvitamin D levels of <75 nmol/L had a significantly higher 10-year hip and major osteoporotic fracture probability using the country-specific fracture risk assessment (FRAX) algorithm. INTRODUCTION: The aim of this study is to characterize the prevalence and seasonal variation of hypovitaminosis D and its relationship to bone metabolism in healthy Hungarian men over 50 years of age. METHODS: We determined levels of 25-hydroxyvitamin D (25-OH-D), PTH, osteocalcin (OC), C-terminal telopeptides of type-I collagen (CTX-I), procollagen type 1 amino-terminal propeptide (PINP), BMD at L1-L4 (LS) and femur neck (FN), daily dietary calcium intake, and the 10-year probability of hip fracture and a major osteoporotic fracture using the countryspecific FRAX algorithm in 206 randomly selected ambulatory men. RESULTS: The mean (range) age of the volunteers was 60 (51-81) years. The prevalence of hypovitaminosis D (25-OH-D, <75 nmol/L) was 52.9%. The mean (range) FRAX hip fracture and FRAX major osteoporotic fracture was 0.8% (0-9.4%) and 3.8% (1.7-16%), respectively. On comparing the vitamin D sufficient to the insufficient group, there was a statistically significant difference between the FRAX hip fracture and FRAX major osteoporotic fracture indexes. There was significant seasonal variation in the vitamin D levels; the lowest levels were measured in winter and the highest in summer. CONCLUSIONS: A high prevalence of hypovitaminosis D and low BMD were observed in the studied Hungarian male population. This is the first study reporting higher 10-year hip and major osteoporotic fracture probability using the country-specific FRAX algorithm in individuals with hypovitaminosis D.

Abstract
 

Objective: To describe a new technique for embryo reduc- tion (ER) in dichorionic triplet (DCT) pregnancies. Methods: In 22 DCT pregnancies, ER to dichorionic twins was carried out at 11.3–13.9 weeks’ gestation by ultrasound-guided la- ser ablation of the pelvic vessels of one of the monochori- onic twins. Results: Intrafetal laser was successfully carried out in all cases, but ultrasound examination within 2 weeks of the procedure demonstrated that the co-twin had died in 11 cases and was alive in the other 11. In the dichorionic group there was one miscarriage at 23 weeks due to cervical incompetence and in the other 10 cases there were two live births at a median gestational age of 35.0 (range 32.2–37.1) weeks. In the 11 cases where both monochorionic fetuses died the separate triplet was live born at a median gestation of 38.0 (range 32.2–40.5) weeks.

Conclusions: In the management of DCT pregnancies, ER to dichorionic twins by in- trafetal laser is an additional option to the traditional ones of expectant management, ER by intrafetal injection of potas- sium chloride (KCl) to monochorionic twins or ER by KCl to singleton. © 2013 S.
